Cancellations
Appointments must be cancelled within 24 hours to avoid a $100.00 late cancellation/no show fee.
Insurance & Payments
We accept the following insurance plans:
- 1199
- Aetna
- Cigna
- United Healthcare
- Oxford Freedom/Choice Plus
- Oxford Liberty
- Medicare
- NYS Empire Plan
- Multiplan/PHCS
Most major credit cards are accepted. It is mandatory that we keep a credit card on file due to high deductibles and co-insurance. You will be notified in advance if the card on file will be charged.
Please note: Cosmetic procedures and treatments are not usually covered by insurance.
There is a $300 non-refundable fee for cosmetic consultations. $150 of the fee will be applied to your cosmetic treatment of choice.
